Sports Physicals

If your child receives their yearly well care with Dr Raley, we are able to fill out any school physical forms needed for sports and all activities that your children might patriciate in such as Scouts and Summer Camps.

If we have not seen your child in the last year, we will need to schedule an appointment prior to completing the form. If time is of the essence, please let us know, so we can schedule you appropriately.

Dr Raley requires all patients to have a yearly well check.

As a reminder, your child’s immunization schedule continues during the teen years, so it is best to make a yearly well check appointment.
