New Patient Registration
Thank you for your interest in Brian Raley, MD Pediatrics. Before you register with the practice, I encourage you to read all of the material on the web site regarding my practice philosophy, availability, insurance plans, and so on. For an optimal doctor-patient relationship, both of us should feel that it is a “good fit.” Also, click above to check out the Online Patient Services section to see all of the exciting ways our state-of-the-art technology will increase your convenience as parents.
To be a new patient you must be willing to immunize your child. We require all patients in the practice to be immunized.
Self-registration will not immediately enroll you as a patient of the practice and does not legally obligate us to provide care. Once you've completed registration, please contact our office so we may begin the process of transferring your records and verifying insurance coverage (if necessary).
Attention CURRENT PATIENTS of Dr. Raley's, please do not register. You already have access to the Online Patient Services! Go to the Patient Login link to start accessing your information. If you are unsure of your Login Name (home number) and Password (insurance ID), please call the office and we'll be happy to provide that information to you.
